Passport

• Your passport must be signed and has at leat two empty pages for visa and valid for more than 6 months.

Photo

• One (1) photo is required.

• Photo must be 2×2, color with a plain light or white background and show a full front view of face.

• Please do not staple your photo on application, use paper clip.

Airline ticket

• Please send a copy of your round trip airline tickets, itinerary or e-ticket.

Application Form

• Complete online form and send it to Capitol Visa along with requirement documents. (on the left Menu).

Customer Information Form

• All applicants must complete and submit the customer information form. (on the left Menu)

Business Letter. The letter must explain all the following:

• Nature of business to be performed

• Name and address of reference to be visited

• This letter should request Multiple Entries visa if required

• Guarantee of return transportation and sufficient funds for the visit

• The letter should be signed by an officer of the company other than the applicant.

Apporval letter from Uzbekistan

• Approval letter from your host or agency from Uzbekistan approved by the Government.

Diplomat Letter

• Letter from your Department or Agency, stating the purpose of travel, dates of the trip, and contact person in Uzbekistan.
